The high reliability journey requires a commitment to think about safety much differently than we have before. Organizations will progress when they stop emphasizing individual error and move toward understanding the larger framework that governs behavior. In health care, individual performance is best understood within a system defined by inherent risks. Our goal as safety leaders is to design effective mitigation strategies based on an understanding of how system design and human performance interact to create this risk. An essential piece of safety culture is establishing a just culture in which we evaluate individual actions within the larger organizational context.
